I understand that a non-refundable deposit of $55 is required and will be applied toward the overall design fee. Upon submission, one of Tunnel Vision’s design team consultants will review the request, design specs and contact me to discuss uniform/apparel details, mock-ups, the non-refundable deposit and cost of the project.

All designs and images created are and shall remain the exclusive property of Tunnel Vision, LLC unless otherwise expressly agreed to in writing outside of this agreement. Any unauthorized use, reproduction, distribution, modification, or transfer of said materials shall be deemed a material breach of this agreement and may result in legal action.
